Collingham Medical Centre
With patients' needs at the heart of everything we do, our website has been designed to make it easy for you to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you’ll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.
The Collingham Medical Centre is being inspected by the Quality Care Commission (CQC) on Tuesday, 28 July 2015. CQC would really like to hear what you think about the care you receive here. If you would like to talk to our inspector privately and in confidence, please let Julie Reid know or contact the CQC directly on 03000 616161.
You can also tell CQC about your experience of care at any time online at www.cqc.org.uk/public/sharing-your-experience.
Get Well, Keep Well
Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.
The surgery has automatic doors, internal and external and a portable induction loop system is available. Baby changing facilities are available in the disabled toilet.
Proactive Care Programme (PCP)
The NHS has introduced a new service for people who are more at risk of an unplanned hospital admission in an emergency situation and who would be likely to benefit from more tailored, active support from their GP surgery. We believe personal care planning could help improve your health and wellbeing, as well as help to reduce the risk of unplanned hospital admissions. We will be writing to patients identified as potentially benefiting from this programme.
