Please bear with us …..

Tuesday, 16 May 2017

The practice has not had access to Computer Systems since Friday, 12 May 2017.   The computer systems for whole mid-Nottinghamshire area started to be restored yesterday and we are expecting engineers at some point today, they do have over 50 sites that they have to visit and are working as safely as fast as they can.

We continue to support as many people as we can and are focusing our resources on urgent advice for today and services which must be done today to support our patients health otherwise we ask that you call back later in the week for us to help you.

We will endeavour to support as many patients as possible today within the constraints of no clinical record.   Please bring as much information with you to the appointment as possible.

Repeat Prescriptions

If you have ordered a repeat prescription online or side slip since 2.00 pm on Friday 12 May 2017, we will not have been able to process this request for your nominated pharmacy.   Please allow an additional 48 hrs for collection (4 days).   If this is going to pose urgent problems, please ask at reception.

Thank you for your co-operation.

During 2015 the NHS has asked that each practice updates patients records to ensure you have a named accountable/usual GP.  We have done this.  This does not prevent you from seeing any GP.   If you would like to know who your accountable/usual GP is, please ask when next in surgery.

Collingham Medical Centre

With patients' needs at the heart of everything we do, our website has been designed to make it easy for you to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you’ll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.

Get Well, Keep Well

Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.

Access

The surgery has automatic doors, internal and external and a portable induction loop system is available. Baby changing facilities are available in the disabled toilet.

Proactive Care Programme (PCP)

The NHS has introduced a new service for people who are more at risk of an unplanned hospital admission in an emergency situation and who would be likely to benefit from more tailored, active support from their GP surgery.   We believe personal care planning could help improve your health and wellbeing, as well as help to reduce the risk of unplanned hospital admissions.  We will be writing to patients identified as potentially benefiting from this programme.
