Proof of COVID-19 vaccination status
Patients can access their COVID-19 vaccination status though the NHS App. Patients are being advised by Gov.uk that they do not need to contact their GP surgery about their COVID-19 vaccination status.   

Currently, the Booster is not required for your COVID passport (it is your primary doses) which define your COVID passport.   Vaccination information is available on the NHS App to view.

FREE Lateral Flow Test Devices are no longer available from the Collingham Pharmacy.   They are available to purchase. 

If you are vulnerable or are advised to take a test by the NHS you can order tests on line at gov.uk

e-Referral

doctors_bag3e-Referral is a service that allows you to choose your hospital or clinic and book an appointment with a specialist.  When you and your GP agree that you need to see a specialist, you will be able to choose from a number of service providers the five closet hospitals are:  

  • Newark General Hospital
  • Kings Mill Hospital
  • Queens Medical Centre
  • Nottingham City Hospital
  • Lincoln County Hospital  

The Benefits of e-Referral

  • You can also choose the date and time of your appointment.
  • You experience greater convenience and certainty. With the old system, your hospital or clinic chose the date and time of your appointment. This may not have been convenient for you. With e-Referral, the choice is yours.
  • There is less chance that information will get lost in the post as information is shared electronically.  

Booking your Appointment

Once your referral has been processed by the practice, we will send the booking information to you.  You can choose your preferred healthcare provider and book an appointment by telephone or online.
