CHOOSE & BOOK Choose & Book is a new service that allows you to choose your hospital or clinic and book an appointment with a Specialist. From February 2006 Collingham Medical Centre has been operating Choose & Book. When you and your GP agree that you need to see a Specialist, you will be able to choose from five hospitals or clinics: Newark General Hospital Kings Mill Hospital Queens Medical Centre Nottingham City Hospital Lincoln County Hospital The Benefits of Choose & Book ¨ You can also choose the date and time of your appointment. ¨ You experience greater convenience and certainty. With the old system, your hospital or clinic chose the date and time of your appointment. This may not have been convenient for you. With Choose & Book, the choice is yours. ¨ There is less chance that information will get lost in the post because more correspondence takes place through computers. Booking your Appointment During your consultation, the doctor will discuss which hospital you would like to attend. The doctor will then direct you to reception where the Secretary will set up the request and book your appointment for you. In the event that the Secretary is unable to book your appointment at the time of your consultation, you will be issued with a unique reference number and password along with the telephone number of the Choose & Book National Appointment Line who will be able to book the appointment for you.
